SUMMARYUnder the supervision of the Post-Acute Care Director and Medical Director, the SNFIST acts as the primary transitional care provider and facilitates members' individualized plan of care in collaboration with members, families, caregivers, and other providers of the interdisciplinary team.The SNFIST uses their clinical judgment to determine a member's needs at the skill nursing facility. The SNFIST provides hands on care, support, guidance, education, and acts as a liaison between the member's Primary Care Physician and specialty physicians to maximize health outcomes and prevent unnecessary hospitalization. The SNFIST also supports a variety of care programs, outreach, education, quality improvement, and other initiatives as needed.ESSENTIAL DUTIES AND RESPONSIBILITESReports to assigned skilled nursing facility Monday through Friday for at least 5 hours a dayExpected to manage after hour calls and weekend calls for the SNF patientsComplies with NMM, department specific policies and procedures, NMM Nurse Practitioner Standardized Procedures, other licensure specific protocols and lawsWill follow a caseload of high-risk geriatric patients in the post-acute care environmentResponsible for up to 35 patients. Develop and implement a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement.Receive handoff reports from hospitalist discharging patients to the SNFCollaborate with hospitalists and specialists on plan of care to maintain continuityUpon discharge from the assigned facility, SNFIST must provide a discharge summary to PAC home Nurse Practitioner teamThe SNFIST will assess the health status/needs of the patient/family by means of comprehensive health history, physical exam, thorough medication reconciliation, and ongoing needs assessment.The SNFIST will initiate selected therapeutic and diagnostic procedures, consultations, and laboratory examinations, prescribes and alters medication regimens, interprets findings and alters of plan of care according collaborative practice agreement and protocols.The SNFIST will provide the patient and family on-going contactThe SNFIST will provide teaching and counselling in the areas of health promotion, maintenance, disease and chronic illness exacerbation, and management of acute/chronic illnesses.The SNFIST will establish rapport with the patient's family and patient themselvesThe SNFIST will consult a physician (Dr. Lee/Dr. Wang) in a timely mannerThe SNFIST will advocate for patient/family and support patient's rightsThe SNFIST will collaborate with all case management teams, ICM and ACM to optimize patient outcomes and satisfactionThe SNFIST will collaborate with the PAC team in the clinic to organize transfers, transfusions, and other necessary planned admissions to our partnered hospitalsWorks independently and keeps program director and medical director informed of members' statusProvides health maintenance, and medication and disease management education to members and families/caregiversCollects, organizes, documents, and analyzes data, synthesizing it into understandable information and provides electronic documentation that meets regulatory requirementsActive participation in case study presentations weeklyEffectively communicates, problem-solves, and maintains productive and effective interpersonal relationships while effectively prioritizingWorks with outside facilities, agencies, and vendors on a routine basis, maintaining positive working relationshipsSupports members and their families/caregivers, and provides compassionate careOther related duties, as assigned`Benefits:Benefits will begin 90 days after employmentHealth insurance will be providedAll licensing renewals will be covered such as DEA, NP licensure and other essential licenses to practiceMalpractice insurance will also be coveredQUALIFICATION:Current California Nurse Practitioner LicensureCurrent California Nurse Practitioner furnishing LicensureCurrent National Board Certification (AACN, ANCC)Valid DEA numbers, NPIActive malpractice insuranceAt least 3 years of experience as a Registered Nurse#HPIND
