Day Hours per Shift: 8 Experience: Minimum of 3 years of inpatient
case management experience is required. License: Active California
license is required. Certifications: Relevant certifications are
required. Must-Have: Knowledge of HMO and Medicare for inpatient,
home health, and skilled nursing facility settings is essential.
Epic experience is necessary. First-timers are accepted. Flexible
to float within the scope to meet facility needs. Description: The
role involves managing patient care across various settings.
Candidates must demonstrate expertise in care coordination and
discharge planning. Familiarity with Centers for Medicare and
Medicaid Services guidelines is essential. The position requires
utilization of InterQual criteria and Milliman guidelines.
Knowledge of HIPAA guidelines is necessary. Experience with DRG
coding and ICD 10 coding is essential. Candidates should be
well-versed in national safety goals and core measures. The daily
census is approximately 255 patients, with a patient ratio
experience of 15. The patient age groups include adolescents,
adults, and geriatrics. Interdisciplinary support will be available
from social services and transportation. Candidates should be able
to adapt to different patient diagnoses and special equipment as
