Job Description:

SUMMARYUnder general supervision, following established procedures, assist and direct patients in an empathetic and caring manner, efficiently and timely, and completes appropriate documentation, as needed. Provide a point of contact for member, doctors, hospitals, etc. Ensure Inquiries are promptly, efficiently and courteously handled and distributed efficiently and promptly.ESSENTIAL DUTIES AND RESPONSIBILITIESWorks under the general supervision of the Department Manager Post-Acute CareComplete post Annual Wellness Visit (AWV) instructions from Nurse Practitioners: request authorization(s) and medical records from imaging facilities or doctors officesStepping in to assist AWV callers and schedulers when there is an influx of patient to be completedSkilled Nursing Facility (SNF) assignments: verify that the facility has received all discharge summaries and instructions for each specific patientMedication Reconciliation Process (MRP) assignments: responsible for gathering data and confirm completion status of each patient case or recordConduct patient calls to introduce newly implemented programs of PAC and/or Allied Pacific IPATransition of Care (TOC) assignments: Conduct follow-up care coordination on patients to ensure post-discharge follow-up is carried out such as scheduling appointments, request authorizations, receiving medications, and schedule DME deliveriesExpectation of a minimum of 10-15 TOC cases per month, with potential up to 30 cases monthlyTranslation assistant whereas needed in the programs above and for specialist officesFamiliar with Microsoft Office: Word, Excel, OutlookAll other duties as assignedQUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have ability to plan, prioritize, and complete tasks under stress with frequent interruptions. Working experience with Microsoft Word and Excel a must. Must be able to type 65 wpm.EDUCATION and/or EXPERIENCEAssociates Degree (AA) required, Bachelors degree preferred or at least 12 months related experience and/or training; or equivalent combination of education and experience.LANGUAGE SKILLSAbility to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Preferably fluent in English and Spanish or English and Chinese (Mandarin and/or Cantonese).#LPIND
